Responsibilities
- Assist the licensed practitioner in identifying patient needs, priorities of care, and goals based on patient factors such as age, background, and beliefs.
- Gather, observe, record, and communicate patient responses to nursing interventions; modify the plan of care in collaboration with the licensed practitioner.
- Demonstrate competency in department-specific functions and contribute to the evaluation and adjustment of the care plan.
- Function as a member of the multidisciplinary team to implement an integrated, patient-centered care plan; delegate nursing interventions to others with appropriate skills as needed.
- Identify early warning signs of change in a patient’s condition; implement nursing interventions and prescribed regimens in a timely and safe manner, with accurate documentation.
- Communicate progress and education needs to patients/families and other team members; assist in discharge planning and provide appropriate education and counseling.

Requirements
- Active LPN license in Minnesota (current and in good standing)
- Ability to perform nursing assessments, care planning, and documentation within licensure requirements
- Comfortable working in an inpatient care setting and collaborating with a multidisciplinary team
- Availability for a 13-week travel assignment with 36 guaranteed hours per week
- Strong communication and teamwork skills; ability to coordinate with patients, families, and staff
